The UIL has released the new requirements for Physicals in marching band and UIL athletic activities due to the current COVID-19 pandemic. All students participating in UIL marching band and/or UIL athletic activities will be required to complete the UIL Medical History Form. Any “yes” answer to any of the questions 1-6 on the Medical History Form will require a student to receive further medical evaluation, which may include a physical exam, and receive written clearance from a physician, physician assistant, chiropractor, or nurse practitioner before any participation in UIL practices, games, performances, or matches. Only those students with “yes” answers to those questions are required to receive written clearance through further medical evaluation.
Any participant in athletics or marching band, in any grade level, who has not previously completed a Pre-Participation Physical Evaluation, and been cleared for participation, will be required to complete the Medical History Form as noted above, as well as a Pre-Participation Evaluation prior to participation in any UIL practice, games, performances, or matches.
Eden CISD has partnered with the Frontera Health Clinic to provide physicals at no cost to you. You can call and schedule an examination with them at 325-869-8811. If you would rather schedule your appointment with your personal physician, that is acceptable as well.
I am including the Pre-Participation Evaluation and Medical History form in this letter. Please take the form with you to your examination. We will require that every student that will participate in UIL athletics and/or Marching Band to have the Medical History on file. If you did not answer yes to questions 1-6, please complete the Medical History form and return it to Coach Avants.
